Instructions

  1. Clinicians requesting replacement of a failed implant must complete a BIOMET 3i Complaint Report. Clinicians requesting restorative benefit must complete a BIOMET 3i Restorative Reimbursement Form. The forms are available for downloading at the links to the right and can be completed online before printing. Please make a copy of your form for your files.
  2. Please clean and sterilize the product(s) before mailing. Non-sterilized devices may be considered biological hazards based on current United States Postal regulations
  3. Please send the sterilized product(s) and original forms to the Warranty Department at BIOMET 3i. Please use a padded mailer to ensure the product is received by BIOMET 3i.
